Yes, currently if one person wants the Cheers Program, everyone in the same cabin 21 years of age or older must purchase. It was originally intended to cut down on the abundance of sharing of drinks between those on the program and those who aren't - with the highest percentage of rule breakers being ones coming from the same cabin.
The good news is that Royal Caribbean just last week released their updated alcohol program and they went from the same thing (if one purchases in the cabin, everyone in the cabin must purchase) to now NOT requiring everyone in the same cabin to purchase. So you can bet Carnival is going to have to look at this even closer now.
